FREE TAX PREPARATION…. MUST BRING THE FOLLOWING:
  • Picture ID
  • Social Security Cards (or ITINs) and dates of births for the taxpayer, spouse, and children and any other dependents you are claiming.
  • W2 form (Document provided by your employer listing your earnings and taxes paid.) 1099 Forms (Document listing your different sources of income. Most commonly used by people working under contracts.)
  • Copy of 2011 Income Tax Return if possible.
  • Bank account number, routing number. (Bring a voided check.)
  • First come, first served basis. Additional appointments are available at different dates

Disability Evaluation Under Social Security,(also known as the Blue Book), has been specially prepared to provide clients and other concerned professionals with an understanding of the disability programs administered by the Social Security Administration. It explains how each program works, and the kinds of information a health professional can furnish to help ensure sound and prompt decisions on disability claims.

The Adult and Childhood Listings of Impairments are included in this publication. These listings are just part of how we decide if someone is disabled. We also consider past work experience, severity of medical conditions, age, education, and work skills.

Homeless Veterans

If you have veterans who are interested in filing claims for Veterans compensation and/or pension please refer them to my email address and or cell phone # 973-704-3865. I am able to take claims for these benefits over the phone as well as provide advice on other benefits that they may be eligible for. I also conduct training for shelter/agency staff in VA benefits and will do onsite visits to take claims or answer questions to groups of veterans. If they require assistance in getting into VA healthcare programs such Healthcare for Homeless, the Domiciliary program or transitional housing they may call me VAMC Lyons directly at 908-647-0180 x14217
